420 People, Three HA Treatments: Is the Skin Booster Finally Getting Some Evidence?

There are a lot of things called “skin boosters” these days. Some have tiny studies, some have case reports, and some have beautiful before-and-after photographs without much clinical evidence behind them.

So when I came across a 420-person randomised controlled trial of a non-cross-linked hyaluronic acid injectable, I paid attention.

HA isn't new. What is interesting is seeing a familiar material put through a much more substantial clinical test.

What did they actually study?

420 participants.

Yes, I noticed the number too. 😂

But no, this isn't a secret Chinese beauty code. It's simply the number of people enrolled in the trial, and for an aesthetic study, that's actually the interesting part.

A 2026 multicentre randomised controlled trial published in Dermatologic Surgery evaluated a novel non-cross-linked sodium hyaluronate gel for facial skin rejuvenation in 420 participants.

Participants were randomised 2:1 into treatment and control groups. The treatment group received three full-face intradermal injections at three-week intervals, while the control group received no intervention. The researchers then followed participants for six months.

The primary outcomes included blinded assessments using the Global Aesthetic Improvement Scale (GAIS) and a composite measure of skin dryness and dullness. They also assessed skin hydration, elasticity, fine lines and roughness.

The results were surprisingly large

The treatment group had a 90.63% improvement rate on the Global Aesthetic Improvement Scale, compared with 3.65% in the control group.

For the composite dryness and dullness measure, the improvement rates were 83.20% versus 3.65%.

Both differences were highly statistically significant, with P < .0001. The researchers also reported significant improvements in skin hydration and elasticity, with the reported effects sustained for six months. Peak efficacy rates for fine lines and roughness reached 51.59% and 50.00%, respectively.

The reported adverse events were mild and transient, with their incidence decreasing over time.

Those are substantial numbers.

But they need some context.

What does “skin rejuvenation” actually mean here?

The study wasn't showing that the treatment reversed biological ageing or permanently made the participants younger.

It was measuring specific aspects of facial skin quality: hydration, elasticity, dryness, dullness, fine lines and roughness.

I actually prefer this way of looking at these treatments.

“Skin rejuvenation” can sound like one enormous claim, when in reality skin has many different measurable characteristics. If hydration improves, we can measure that. If elasticity changes, we can measure that too. If fine lines or roughness improve, those can be assessed separately.

That makes the discussion much more useful than simply saying that something “rejuvenates” the skin.

And this wasn't a conventional filler

The product was non-cross-linked HA, which is different from the cross-linked HA used in traditional dermal fillers.

Cross-linked HA forms a more persistent gel structure and is primarily used for volume and contouring. Non-cross-linked HA is being investigated in a different context, including skin quality rather than structural volume.

There is a reasonable biological basis for this. Hyaluronic acid is naturally present in the extracellular matrix and has an important role in water retention and tissue structure.

But biological plausibility is not clinical evidence.

The interesting part is that this study actually tested a specific non-cross-linked HA formulation in hundreds of people.

Why 420 people matters

Aesthetic medicine has a lot of small studies. Ten people, fifteen people, twenty people, a few before-and-after photographs and a short follow-up can generate an interesting signal, but they don't tell us nearly as much as a properly designed controlled trial with a substantially larger sample.

This study enrolled 420 participants, used randomisation and blinded outcome assessment, and followed participants for six months.

That doesn't make it perfect, but it does make the evidence considerably more useful than a tiny uncontrolled case series.

But this is not proof that all HA skin boosters work

This is probably the most important qualification.

The trial tested one specific non-cross-linked HA formulation, using one particular treatment protocol, in a Chinese population.

There are many different HA injectables with different formulations, concentrations, molecular characteristics, manufacturing processes and injection techniques. The results from this study shouldn't automatically be applied to all of them.

The control group also received no intervention, rather than a placebo injection or another active treatment. That means the study cannot completely separate the treatment effect from everything associated with undergoing an aesthetic procedure, although the blinded assessments strengthen the design.

There is also a potential conflict of interest worth noting. Two authors were affiliated with Hyamed Biotechnology, so independent replication would make the findings more convincing.

None of that invalidates the study.

It simply tells us how far we can reasonably take the result.

So what does this study actually tell us?

A fair conclusion would be that this particular non-cross-linked HA gel was associated with significant improvements in several measures of facial skin quality compared with the no-treatment control, with benefits reported through six months.

That's quite different from saying:

“HA skin boosters work.”

The evidence is product-specific.

And that's okay.

In fact, I think that's exactly how aesthetic research should work. We don't need one study to prove that an entire category of treatments works. We need individual products and protocols to be tested properly, so that over time we can work out which ones actually have reproducible effects.

This is the kind of study I want to see more often

What interests me isn't that HA has suddenly become revolutionary.

It hasn't.

It's that a familiar material has been tested in a 420-person randomised controlled trial, with defined outcomes and six months of follow-up.

I'd like to see more studies like this, ideally with independent investigators, different populations, longer follow-up and comparisons with other established skin-quality treatments.

Because the goal isn't to prove that every injectable is amazing.

It's to work out which ones actually earn the attention they get.

And for this particular HA formulation, we now have something much more useful than another before-and-after photograph:

a meaningful clinical signal worth investigating further.
